Support Coordinator
As a Support Coordinator, you'll help implement and manage all supports outlined in NDIS plans, enhancing participants' abilities to navigate and engage with their community, and fostering independence.
You will work closely with mainstream services and the NDIA to ensure success and provide regular reports.

Skills & Experience

At least 12 months of experience in support coordination or extensive knowledge of NDIS

Relevant qualifications or equivalent experience in case management, disability, or community services
